Travel Card Application
If you have departmental approval to obtain a Travel Card but have not yet submitted a Travel Card application, please fill out this DocuSign form to submit an application. You will need your responsibility center number to complete the form. Once your Travel Card application is approved, you will receive login and password information for a brief online Travel Card training module. Completion of the Travel Card training module is mandatory.
Travel Card Acceptance Form
A DocuSign link to the Travel Card acceptance form will be available to you immediately after you have successfully completed the brief online Travel Card training module mentioned above. Completion of the acceptance form is mandatory and requires the same signatures that your Travel Card application required. A representative from the Payment Processing department will check to verify whether you have completed an acceptance form prior to sending you a Travel Card. You will not receive a Travel Card until you have completed a Travel Card acceptance form.
Travel Card Change Request Form
To request a change to your Travel Card, please complete a Travel Card change request form through DocuSign. The Travel Card change request form requires the same signatures that your application and acceptance form required. Once your Travel Card change request form has been completed and signed, DocuSign will forward it to the Payment Processing department. A representative from the Payment Processing department will then review your request and will send you a status update.
In rare occurrences, a cashier could decline your ability to transact if the name on your card does not match your driver’s license.
